WHAT TO EXPECT ON YOUR FIRST VISIT
Prior to our appointment, you will fill out a questionnaire that reviews your medical history, physical activity, and current nutrition habits. Our initial appointment is approximately 60-90 minutes.
Together, we will create a plan that provides tips and tools in making small but realistic changes that will take you steps closer in reaching your long-term goals. At follow up appointments, we review the changes that you made well and then further identify barriers to success and determine the best course of action.

THE INITIAL CONSULTATION WILL INCLUDE:
• An initial Comprehensive Nutrition Assessment, which is obtained through various health profiling questionnaires, review of recent lab reports, diet recalls, food records or overview of your typical intake.
• A bioimpedance analysis to measure and assess your hydration status, fluid compartment balance, cellular resiliency, and computation of body composition.
• An assessment of the overall adequacy of the diet. This may also focus on specific nutrients that may need modification (i.e., sodium, fiber).
• Instruction is individualized, addressing the primary reason for referral and the overall adequacy of the diet.
• To meet your needs, goal-setting, self-management training, and development of a nutrition protocol are completed.
• If applicable, a letter will be sent to the primary care physician who completed a nutrition referral, discussing the assessment and goals determined.

THE FIRST FOLLOW-UP VISIT WILL INCLUDE:
• Progress with goals and measurement of dietary compliance is completed, using diet recalls, food records, overview of your typical intake, weigh-ins, labs or subjective discussion.
• Based on your progress, further nutrition instruction may be needed.
• Focus is placed on goal setting for your next visit.

SUBSEQUENT FOLLOW-UP VISITS WILL INCLUDE:
• Current weight, bioimpedance analysis, or labs of nutritional significance are discussed.
• Any problems or concerns regarding the established nutrition plan or goals is addressed.
• If goals are met, new goals are established as necessary.
• A letter will be sent to the primary care physician who completed a nutrition referral, discussing your progress. |
